At Rosmed, we’ll work to understand your needs throughout your life transition to ensure that you feel supported at all times, weather it’s school, University or into a new home. we supports participants in their life stage transitions by providing a range of services including
- Life transition planning
- Build participant’s capacity and capability to understand their plan.
- Coordination of supports and services in line with a participant’s wishes and
